I have two suggestions : one technical in nature, the other one maintenance.
Technical Have you tried to tweak your NVIDIA driver settings? Go to: /Manage 3D Settings /Multi- Display - Mixed CPU acceleration Change to: /Single Display Performance Mode
Maintenance Crack open your case and check all of your connections. Since you built it, you should know your way around it. Sometimes a loose connection or even the dreaded dust-bunnies can create issues like this.

Whenever I have a bad lag problem I usually uninstall, re-install, make sure I set the UO settings to defualt, run the game, bring up task manager, see what all is running in the background, and end the process if it's unnecessary. I know this isn't much of an answer, but I'm not very tech savvy. It's just what I do when I have the problem. If the game is installed correctly then it shouldn't lag unless something is in the background (connection based or simply processor based) making it do so.
And sometimes your CPU just isn't compatible with it. I know for a fact this HP laptop I'm using doesn't agree with UO 100%. It runs fine, but ever couple of minutes I'll spike a ping of 800 or so, only for about a second or two (usually long enough to get me killed). I know it isn't my connection because I've played with another laptop and never had the problem. So keep that in mind as well.

I decided to reinstall the Nvidia drivers to see if I could notice any changes... Now I get BSOD while loading Windows! The error reads "INTERNAL POWER FAILURE", this even happens when trying to boot in Safe Mode. Occasionally it will boot normally... with no real explanation. Luckily, I goofed awhile back and installed a different version of WinXP on the same drive.... and guess what? This version boots just fine. To clarify, Windows XP Home edition blue-screens 9/10 times, Windows XP Pro loads with no problem. So now I load XP Pro, and check the crash logs via WhoCrashed utility. There are 2 entries, one points at ntoskrnl.exe with the error "INTERNAL_POWER_ERROR", a second entry points to nv4_disp.sys with the error "KERNEL_MODE_EXCEPTION_NOT_HANDLED_M". The WhoCrashed summary states this is all due to nv4_disp.sys causing the initial problem and suggests updating the driver. My neighbor loaned me his Windows XP installation disk. I booted from disk and using the Recovery Console, replaced the ntoskrnl.exe as a test. Still getting BSOD. Google the nv4_disp.sys and you will find many people out there with similar problems as mine, which apparently only seem to be fully resolved by reinstalling Windows, replacing motherboard, or both. Will try more testing this evening. Any suggestions would be appreciated.

Just reload windows. That's what I do. Use Ninite to reload all your programs.

Since my OS bit the dust, and my install cd is in a storage unit 80 miles away, I decided to take this opportunity to buy a new HD and upgrade to a 64-bit Windows. I will be back on the server as soon as I can find my game disks. Those are probably in storage too so I might have to wait a few weeks. If I get really bored, I will buy a new cd key on steam Thanks for the help guys!!

I love Comcrack even though they are overpriced as an ISP but I get steady connection anywhere below 100 ping.
Oh wait, I'm on a mac so I don't have this sort of problem (lag spikes), except lack of games for my platform. LOL

Very often causes overheating of the system lag.It is important to know the CPU temperature.The most famous program is CoreTemp.A game of Call of Duty can cause the processor to raise 45 degrees. My processor is (Intel Core i5-2550K@3.40GHz}.You may need to change the thermal paste(It does not cost much) on the CPU heatsink.Third way it is most popular is to put additional fan.I hope I've helped a little.
